Family Medical Care Community Health Center (FMCCHC) prides itself on being a comprehensive health care facility with clinical staff committed to providing the best possible care with the utmost respect and compassion for each of its patients.
FMCCHC includes family health, women’s health, pediatrics, behavioral health, dental care, and vision care services to the entire community, regardless of age, income level, insurance status or residence. We accept patients with Medicare, Medicaid, and third-party insurances as well as provide a discount program to eligible individuals.
A lab is available at all three of our Family Medical Care locations. No appointment necessary!
As a Federally Qualified Health Center (FQHC), our patients have access to the 340B program which provides substantial savings on medications ordered through our providers. For convenience to our patients, our Family Care Pharmacy is an exclusive 340B pharmacy and only patients of Family Medical Care are eligible for the 340B program. CHANGE, Inc. has various contracts with pharmacies throughout the Ohio Valley for the 340B program including Value Leader Pharmacy, Tri-State Pharmacy, Follansbee Pharmacy, Citizens Drug Store of Chester, and New Cumberland RX Center.
Transportation is available to and from medical appointments at Family Medical Care locations. To request transportation, call our Transportation department at 304-797-7733 x1400 at least a week prior to your appointment (Exceptions can be made if it is an emergency).
Childcare Services are available during appointments at Family Medical Care. To request childcare services, call the Health Benefits Coordinator at least 24 hours prior to your appointment.
Family Medical Care and Family Care Pharmacy accepts most insurance plans, cash, check or credit card, and offers payment plans for patients who cannot afford to pay their full medical bill upfront. Contact the Business Office at 304-797-1210 for more information.
Family Medical Care offers patients a sliding fee discount program that includes discounted services regardless of any patient’s ability to pay. The program is based upon Federal guidelines and takes into account all persons living in the household and household income. For information or an appointment, please contact our Health Benefits staff at one of our locations.
Family Medical Care offers automated courtesy calls and text messages to remind you of your upcoming appointments. Ask our staff to learn more!
Family Medical Care offers a Patient Portal that is open to our patients. You can refill prescriptions, make appointments, ask a question, and view your office visit summaries all from the convenience of your computer, tablet or phone. Please visit one of our FMC locations to sign up and have more access to your healthcare.

In July 2007, Family Medical Care Community Health Center was designated as a Federally Qualified Health Center (FQHC), and is one of the 38 community health centers in West Virginia.
Family Medical Care is made possible through funding from the WV Bureau of Public Health Primary Care Division, Third Party Insurance, Private Pay, Private Foundations, and is being presented with financial assistance as a grant from the West Virginia Department of Health and Human Resources and through federal dollars from the Department of Health and Human Services Health Resources and Services Administration, and the Community Service Block Grant. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
