What We Do

Case Management provides assistance to individuals and/or families by addressing key issues and providing supportive services to overcome barriers to self-sufficiency.

CHANGE, Inc.’s case managers work with individuals to help them develop goals, overcome obstacles to self-sufficiency, and build the necessary skills to achieve financial stability.

How To Receive Service

Please call CHANGE, Inc. to schedule an appointment at one of our office locations. You will be asked to provide a copy of your photo ID and fill out an intake form. Based on the results, you will be referred to the appropriate staff and/or resources depending upon your immediate needs.
