As part of its requirements under the Community Service Block Grant and Human Resources and Services Administration, CHANGE, Inc. conducts a Needs Assessment of the Community and its partners every three years. The responses help to determine the services needed in the community, and resources available. CHANGE, Inc. asks for your help in completing the surveys.
Two versions of the survey are available: the Community Needs Assessment, which is to be completed by residents of the five counties CHANGE, Inc. services (Hancock, Brooke, Ohio, Marshall and Jefferson); and the Stakeholders Needs Assessment, which is to be completed by organizations servicing the five counties. The surveys can either be completed online, or downloaded and emailed or mailed to CHANGE, Inc.
